Introduction:

Lincoln Community Hospital and Care Center is a 15-bed Critical Access Hospital with a 35-bed long term care unit; 3 provider-based clinics, 1 “walk-in” clinic, 1 outpatient clinic that conducts 16 specialty clinics; PT/OT/SLP services; and a home health/hospice agency.

Job Description:

Health Information Management (HIM) Director/Coder is responsible for the operation of the HIM Department for Lincoln Community Hospital and Care Center. This includes medical record management for the Hospital, the Care Center, the Clinics, and Home Health/Hospice. The Director/Coder is responsible for evaluating and improving processes in the department, and for storage and retrieval of health information and data. The Director/Coder codes patient records following industry established guidelines and best practices. The Director/Coder assures that polices and operating procedures are consistently administered to manage health information in an efficient and effective manner.

MEDICAL RECORDS – Leads the HIM operations staff to ensure that LCHCC medical records are accurately and efficiently maintained. This includes: developing operating procedures and managing processes for medical record reconciliation with patient admission and discharge, record analysis for deficiencies and completion of deficiencies, scanning paper records into the electronic medical record, managing dictated and transcribed documentation, managing forms and admission packets, managing release of information procedures, and managing medical record retention and destruction according to established guidelines or procedures.

CODING – Reviews, interprets, codes and abstracts medical record information into diagnostic and procedural codes (ICD 10 CM and CPT) to maintain accurate databases, for reimbursement,
and for reporting as required. Manages coding volumes by monitoring discharged not final billed reporting, coding denial work queues, coding edit work queues, and distribution of work among coding staff.

PERSONNEL – Assures orientation and training programs are in place to ensure ongoing staff competence and monitors and evaluates staff performance according to facility guidelines. Interacts with medical staff members and other department leadership and staff in a professional and appropriate manner.

PERFORMANCE IMPROVEMENT/REGULATORY – Performs interdepartmental and intradepartmental quality/performance improvement activities and works with facility leadership to meet or exceed financial goals and objectives. Complies with regulatory standards for medical record management to include state and federal requirements and complies with requirements for accurate coding, abstracting, and billing claim submission.

EDUCATIONAL REQUIREMENTS: Registered Health Information Technician (RHIT) or Registered Health Information Administrator (RHIA) credential required. Previous HIM management experience highly desirable but will consider new graduate who has passed the RHIT or RHIA examination. Documentation of certification and documentation of continuing education credits required.

KNOWLEDGE AND SKILLS: Must have a good working knowledge of grammar, punctuation, spelling, and word processing skills. Must have above average understanding of medical terminology and disease processes. Must be able to learn and follow established policies and procedures and be able to prioritize work activities to meet deadlines in a timely manner.

PHYSICAL DEMANDS: Finger and hand dexterity to operate computer systems, electronic medical records, and paper records; good verbal, visual, and auditory ability; able to stand, stoop, and stretch when needed to file.

SPECIAL DEMANDS: Must be flexible and well organized; able to prioritize work activities and meet deadlines; perform all assigned duties with minimal supervision; follow instructions and complete duties in a timely manner.

CHALLENGES: Prioritizing the numerous work activities and completing all work accurately while promoting a cooperative environment with the various departments this position interfaces with.

Education Qualifications:
